AN ACT CONCERNING A UNIVERSAL PATIENT INTAKE FORM FOR RECIPIENTS OF BEHAVIORAL HEALTH SERVICES FOR CHILDREN. To require providers of behavioral health services for children to utilize a universal patient intake form.
