STATEMENT OF PURPOSE

This legislation transfers the responsibilities for the health care directive registry from the Secretary of State to
the Department of Health and Welfare, including the existing records.
FISCAL NOTE
The cost to design, build, and implement an advanced care directive database system in the Department of
Health and Welfare is estimated at about $250,000 onetime. The design and buildout can be funded at a 90%
federal share using HITECH funds and 10% state match.
It will cost $500,000 ongoing for operation. The ongoing operations will be a mix of 75% federal share and
25% state match for Medicaid eligible participants and 100% General Fund for non-Medicaid, which averages
out to an approximate 50/50 fund split for ongoing operations in a full year.
Design and buildout: $25,000 General Fund; $225,000 federal funds
Ongoing operations, outreach, and trainings: $250,000 General Fund; $250,000 federal funds
