1 Section 1. MEDICAL EXAMINER INVESTIGATION FORM FOR
2 CHILD DEATHS AGE ZERO TO THREE —— INFORMATION REGARDING
3 IMMUNIZATIONS.
4 1. The medical examiner investigation form for cases for
5 infants zero to three years of age shall include a request
6 for information regarding the date, brand, and type of the
7 decedent’s last immunization. If the decedent received more
8 than one immunization at the time of the last immunization, the
9 information provided shall include all types of immunizations
10 received.
11 2. The department of public health shall amend the medical
12 examiner investigation form for cases for infants zero to three
13 years of age to comply with this section.
14 EXPLANATION
15 The inclusion of this explanation does not constitute agreement with
16 the explanation’s substance by the members of the general assembly.
17 This bill requires that a medical examiner investigation
18 form for infants zero to three years of age shall include
19 a request for information regarding the date, brand, and
20 type of the decedent’s last immunization. If the decedent
21 received more than one immunization at the time of the last
22 immunization, the information provided shall include all types
23 of immunizations received. The bill requires the department of
24 public health to amend the medical examiner investigation form
25 for infants zero to three years of age to comply with the bill.
