3 BE IT ENACTED BY THE GENERAL ASSEMBLY OF THE STATE OF IOWA:

1 Section 1. MEDICAL EXAMINER INVESTIGATION FORM FOR
2 CHILD DEATHS AGE ZERO TO THREE —— INFORMATION REGARDING
3 IMMUNIZATIONS.
4 1. The medical examiner investigation form for cases for
5 infants zero to three years of age shall include a request
6 for information regarding the date and type of the decedent’s
7 last immunization. If the decedent received more than
8 one immunization at the time of the last immunization, the
9 information provided shall include all types of immunizations
10 received.
11 2. The department of public health shall adopt rules
12 pursuant to chapter 17A to amend the medical examiner
13 investigation form for cases for infants zero to three years of
14 age to comply with this section.
15 EXPLANATION
16 The inclusion of this explanation does not constitute agreement with
17 the explanation’s substance by the members of the general assembly.
18 This bill requires that a medical examiner investigation
19 form for infants zero to three years of age shall include a
20 request for information regarding the date and type of the
21 decedent’s last immunization. If the decedent received more
22 than one immunization at the time of the last immunization, the
23 information provided shall include all types of immunizations
24 received. The bill requires the department of public health
25 to adopt administrative rules to amend the medical examiner
26 investigation form for infants zero to three years of age to
27 comply with the bill.
