Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code 1-204.51)
and the Procurement Practices Reform Act (D.C. Official Code 2-351.01 et seq.), enclosed for
consideration and approval by the Council of the District of Columbia is proposed Modification No.
M0013 to Contract No. CW50586 with Umbrella Therapeutic Services, Inc. to exercise option period
four in the not-to-exceed amount of $1,795,909.50. The period of performance is from March 17, 2021
through March 16, 2022.
Under this proposed modification, Umbrella Therapeutic Services, Inc. shall continue
to provide Pre and Post Transitional Care Services (PTCH) for the delivery of services to youth
involved in the juvenile justice system supervised by The Department of Youth and Rehabilitation
Services (DYRS).

The Human Care Agreement identified in Block 10A above, is hereby modified as follows:
1. In accordance with Human Care Agreement No. CW50586, Section D.3, Option to Extend Term of
the Agreement, the Government of the District of Columbia hereby exercise Option Year Four to
extend the term of this agreement from March 17, 2021 thru March 16, 2022 for the NTE amount of
$1,795,909.50. Services to be performed are subject to the issuance of task orders. Funding will
encumbered on each task order.
ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED
Except as provided herein, all terms and conditions of the document is referenced in Item 9A or 10A remain unchanged and in full force and effect.
