PACE-RI operates centers in East Providence, Newport, Westerly, and Woonsocket, providing a comprehensive range of services, including primary and acute care, pharmacy, behavioral health, rehabilitation, home care, transportation, and adult day services. The program has successfully enabled 450 medically complex participants to remain in their homes for an average of four additional years compared to those not enrolled, resulting in significant savings for Rhode Island taxpayers.

The resolution highlights the positive impact of PACE-RI on its participants, noting that the average enrollee is 75 years old with multiple medical conditions, and a significant percentage have behavioral health diagnoses or dementia. The program has served over 2,000 individuals, leading to fewer emergency room visits and hospitalizations among its participants. Furthermore, 86% of those enrolled report that PACE-RI's services enhance their independence and quality of life. The resolution expresses gratitude for the high-quality healthcare services provided by PACE-RI and directs the Secretary of State to transmit a certified copy of the resolution to the organization's Chief of External Affairs, Mr. Thomas Boucher.
