Additionally, the bill mandates that service providers must offer a standardized form for service recipients and their physicians to initiate requests for contract changes. If the service recipient is unable to complete the form, a legal representative may do so, provided they submit appropriate documentation of their authority. The bill also stipulates that service providers may require written proof of admission to a long-term care facility to process these requests. Importantly, the bill clarifies that service recipients remain responsible for returning any leased or rented equipment and for paying any outstanding charges related to unreturned equipment. The act is set to take effect 60 days after its passage.
