Imposes conditions on third party network contracts for leasing of dental provider panels. Authorizes Department of Consumer and Business Services to adopt rules to allow individual or group policy or certificate of health insurance to impose copayment of no more than $5 for primary care visit if necessary to comply with federal requirements for equal coverage of mental health and physical health care services in insurance.
