The act creates the NJ Senior Connection Program, which provides grants to eligible county-based organizations for friendly visitor programs, telephone reassurance services, and digital literacy programs targeting socially isolated seniors. Applications for these grants will be evaluated based on criteria such as the effectiveness of the proposed plans to facilitate social interaction and outreach efforts to engage homebound seniors. Additionally, a second grant program will support senior centers in developing newly established mental health screening programs, with funds allocated for staff training and partnerships with mental health providers.

Furthermore, the act introduces a three-year Social Prescribing Pilot Program, allowing healthcare providers to refer seniors to non-medical community activities to improve their overall well-being and reduce social isolation. The program will be implemented in five selected counties, with participating physicians required to register and follow established guidelines. The bill also mandates the expansion of the Statewide Respite Care Program to provide dedicated funding for respite services for caregivers of dementia patients, with a total appropriation of $1.8 million from the General Fund to support these initiatives.
