This bill mandates the Division of Aging Services within the Department of Human Services to create a unified application form for individuals aged 65 and older, as well as those between 18 and 64 who are receiving Social Security Disability. This form will be used to apply for services from various federal and state-funded programs administered by the division and will include all necessary information to determine eligibility for these programs.

Additionally, the bill stipulates that the division will automatically process applications and enroll eligible individuals in all applicable programs unless they choose to opt out or are deemed ineligible. The Department of Human Services is tasked with adopting the necessary rules and regulations to implement this act, which will take effect three months after its enactment, although anticipatory administrative actions may be taken beforehand.
