These plans must include essential information such as the patient's care team details, postpartum visit schedules, guidance on breastfeeding, reproductive life plans, and management strategies for postpartum depression and other health issues. Healthcare providers are required to offer these planning sessions during the first trimester or as soon as feasible, and they must educate patients on the risks of not having a postpartum care plan.

Additionally, the bill requires that all maternity care facilities provide patients with postpartum care information based on best practices before discharge following pregnancy, pregnancy loss, or stillbirth. This initiative is in response to rising maternal mortality and morbidity rates, particularly affecting minority communities, and aims to ensure that women are informed and prepared to seek necessary medical care during the postpartum period. The act will take effect 180 days after its enactment.
