The form will include details such as the date, the name of the service animal and its owner, an assertion regarding the legality of misrepresenting an animal as a service animal, the state of registration, the function of the service animal, and additional notations about the responsibilities of service animal owners. The form will also require the owner's signature and provide space for assistance if needed.

Additionally, the bill establishes a committee tasked with studying the protection of business operators from requests to accommodate customers with fraudulent or untrained service animals. The committee will consist of three members from the House of Representatives and three from the Senate, who will receive mileage compensation for their duties. The committee is charged with reviewing federal law restrictions and considering potential solutions, including the possibility of requesting federal legislation. They are required to report their findings and recommendations by November 1, 2025. The act will take effect upon its passage.
