The bill amends Minnesota Statutes 2024, section 256B.051, by adding a new subdivision that outlines the authorization process for housing stabilization services for individuals eligible for waiver services. It specifies that after a person-centered plan is signed by an eligible provider, the individual's waiver case manager will determine eligibility for housing stabilization services and approve them within certain limits. These limits include up to 150 hours of housing transition services per transition (with a cap on remote services), up to 150 hours of housing sustaining services annually, a maximum of $3,000 for moving expenses per eligibility span, and annual housing consultation services.
Additionally, the bill requires that any requests for exceptions to these service limits must be submitted by the waiver case manager to the commissioner of human services for approval. The new provisions are set to take effect on July 1, 2025.
