The proposed Modification No. M0018 to Human Care Agreement No. CW100384 seeks to extend the contract with Community of Hope, Inc. for an additional year, from July 1, 2026, to June 30, 2027, with a funding cap of $2,235,937.18. This modification will allow Community of Hope, Inc. to continue providing essential services, including case management, utility assistance, and financial support, to chronically homeless individuals and families, as well as other vulnerable populations experiencing homelessness.
