The proposed Modification No. M07 to Human Care Agreement No. CW94269 with Frontline Community Services, Inc. seeks to exercise the fourth option year of the agreement, which will extend the contract from April 1, 2026, through March 31, 2027, with a not-to-exceed amount of $1,789,140.56. This modification allows Frontline Community Services to continue providing occupancy-related residential expenses for District residents with intellectual and developmental disabilities who are receiving residential habilitation or supported living services that are not covered by Medicaid.
