The proposed Modification No. M0011 to Human Care Agreement No. CW101628 seeks to exercise option year three with Echelon Community Services, Inc. for the provision of case management services under the Department of Human Services' Family Rehousing and Stabilization Program. The modification will extend the contract period from October 1, 2025, through September 30, 2026, with a not-to-exceed amount of $2,637,285.06. This funding has been certified as available in the District's Local Budget and Financial Plan for Fiscal Year 2026.
