Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1-
204.51) and section 202 of the Procurement Practices Reform Act of 2010 (D.C. Official Code §
2-352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M0003 to Human Care Agreement No. CW104624 with MBI Health
Services, LLC to exercise option year two in the not-to-exceed amount of $3,500,000. The period
of performance is from December 28, 2024, through December 27, 2025.
Under the proposed modification, MBI Health Services, LLC will continue to provide mental
health rehabilitation services to consumers enrolled in the District’s public mental health system.

14. Description of Amendment/Modification (Organized by UCF Section headings, including solicitation/contract subject matter where feasible.)
The Government of the District of Columbia hereby modifies Human Care Agreement No. CW104624 as
follows:
A. In accordance with 27 DCMR § 2008 and Section F.3 Option to Extend the Term of the Agreement,
the District hereby exercises Option Year Two (OY2) for the period of performance from December
28, 2024, to December 27, 2025, in the not-to-exceed amount of $3,500,000.
B. Continuation of OY2 services beyond FY25 period of performance, from October 1, 2025, through
December 27, 2025, is pending appropriations/availability of funds under the DBH FY26 budget
which will be reflected in a District issued Purchase Order.
Except as provided herein, all terms and conditions of the document is referenced in Item 9A or 10A remain unchanged and in full force and effect.
