Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1-
204.51) and section 202ofthe Procurement Practices Reform Actof 2010 (D.C. Official Code §
2-352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M0004 to Human Care Agreement No. CW101329 with Innovative
Life Solutions, Inc., to exercise option year two in the not-to-exceed amountof $1,292,830. The
period of performance is from November 1, 2024, through October 31, 2025.
Under the proposed modification, Innovative Life Solutions, Inc., will continue to provide
supported rehabilitative services to consumers enrolled in the District's public mental health
system. These services will help to facilitate and sustain the community tenureofadult individuals
with a diagnosis ofsevere and persistent mental illness who are in need of intensive behavioral
and medical support within a structured and supervised setting,

Explanation: The NTE amount for the option periods is higher than the base year due to the
amendment of section 5701 of Title 22A, D.C. Municipal Regulation, Chapter 57, resulting
increment in the per diem for supported rehabilitative residence from $106.15 to $126.50.

current with its District taxes. If the Citywide Clean Hands Database indicates that the

recovers any outstanding debt as provided under D.C. Official Code § 2-353.01(b):

Speaker of the House of Representatives for Fiscal Year 2025 and Fiscal Year 2026 to fund the costs
associated with the Department of Behavioral Health, Human Care Agreement with Community
Connections Inc, relating to the Supported Rehabilitative Residence Services to facilitate and sustain the
community tenure of individuals who are enrolled with the Department of Behavioral Health and have

A. In accordance with 27 DCMR 2008 and Section F.3.1, Option to Extend the Term of the Human Care Agreement, the
District hereby exercises option year two for the period performance starting from November 1, 2024, through October
31, 2025, in the not-to-exceed amount of $1,292,830.
B. Funding of this Human Care Agreement beyond September 30, 2025, is contingent upon the availability of funds in
DBH Fiscal Year 2026 budget.
Except as provided herein, all terms and conditions of the document is referenced in Item 9A or 10A remain unchanged and in full force and effect.
