Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1-
204.51) and section 202 the Procurement Practices Reform Act of 2010 (D.C. Official Code § 2-
352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M00002 to Human Care Agreement No. CW101973 with Wholistic
Home & Community Based Services, Inc. This modification is to exercise Option Year One in the
not-to-exceed amount of $1,273,611.02 for a period of performance from March 1, 2024 through
February 28, 2025.
Under the proposed modification, Wholistic Home & Community Based Services, Inc. shall
continue to provide residential expenses and services not covered by Medicaid for individuals with
intellectual and developmental disabilities. These services are mandated by the Citizens with
Intellectual Disabilities Constitutional Rights and Dignity Act of 1978, (D.C. Law 2-137, D.C.
Official Code § 7-1301.01 et seq.).

in the District's Local Budget and Financial Plan for Fiscal Year 2024 to fund the costs associated

Upon approval of the District’s local Budget and Financial Plan by the Council and the Mayor,
