Pursuant to section 451 of the District of Columbia Home Rule Act (D.C. Official Code § 1-
204.51) and section 202 of the Procurement Practices Reform Act of 2010 (D.C. Official Code §
2-352.02), enclosed for consideration and approval by the Council of the District of Columbia is
proposed Modification No. M0001 to Human Care Agreement No. CW101416 with Life Stride
Inc. to exercise option year one in the not-to-exceed amount of $2,874,966.60 The period of
performance is from September 29, 2023 through September 28, 2024.
Under the proposed modification, Life Stride Inc. shall continue to provide supported rehabilitative
services to consumers enrolled in the District’s public mental health system to facilitate and sustain
the community tenure of adult individuals with a diagnosis of severe and persistent mental illness
and in need of intensive behavioral and medical supports within a structured and supervised
setting.

B. The total contract amount for option year one shall not exceed a maximum of Two Million Eight Hundred Seventy-
Four Thousand Nine Hundred Sixty-Six Dollars and Sixty Cents ($2,874,966.60). Funding of this contract beyond
September 30,2023 is contingent upon the availability of funds in DBH FY’24 budget.
